The Hazard screen is the home for all information related to a hazard and all devices associated with that hazard. It lists facility information, hazard details, hazard level notes, device information, and tests. This screen is accessible when test work is opened from the control panel or when a hazard device is opened from the Hazard Listing screen.
Hazard Information
The information section at the top of the Hazard screen presents details about the facility and hazard. This section includes 9 prefilled fields, with editing enabled for the 7 fields related to hazard information. Facility Name and Water Acct are not editable fields
Save changes made to these fields, by selecting the "Save" button located at the bottom of the section.
Editable Hazard Fields
Water Use: the object that uses the potentially contaminated water
Level: can be Low or High; determines the risk associated with the hazard
Seasonal: can be ‘Yes’ or’ No’, determines how often the device will be tested; Device testing due dates are determined based on client state and the seasonal setting. This setting can be updated at any time.
Location, Floor, Room No, Bldg : physical location of the hazard in the facility
Hazard Notes
The Hazard Notes section contains all hazard level notes. The Hazard Notes are automatically sorted from newest to oldest, but the order can be reversed by clicking the date column.
There are two types of notes available: quick notes and standard notes. Quick notes are designed to be a quicker way to communicate information. You can create a device quick note by selecting the “Removed” option from the drop-down list. A standard note can be up to 345 characters and is limited to ASCII characters.
If a quick note is selected and a regular note is written, clicking "Save" will combine both the quick note and the regular note into a single entry.
Add a Note
- Select the ‘Add’ button from the option list in the Hazard Notes section
- Select an option for the ‘Create Note’ drop-down menu and/or type your note in the box. Facility level notes have a maximum of 350 characters, including the quick note text.
- Click the “Save” button once you have completed your note. The system will automatically record the author and date, referencing the currently logged-in user and the timestamp.
- After the note is saved it will be visible in the Hazard Notes section. Notes are automatically arranged with the newest at the top of the grid, and a scroll bar is added when notes exceed the box's size.
Hazard Device
The Device section lists all devices for the hazard. A hazard can have more than one device, and each device can be in any of the five device states.
The Device information contains places for a user to enter their unique device identifiers. The four fields are the Barcode(BC), Cross Connection Id(CCID), Serial No(SN), and Device No fields. It is recommended that every device has at least one of the four unique identifier fields.
Device States
There are five device states Active, Proposed, Inactive, Removed, and Replaced. The device state can be edited from the ‘Edit Device’ window on the Hazard screen.
Active: The device is at a facility and currently in use. Size, make, model, and device type are required for an active device and cannot be edited. Devices in an active state will have test work listed on the Control Panel.
Proposed: Device is proposed to exist in the future. Size, make, and model a re not required for a proposed device, though device type is still required and not able to be edited. Size, make, and model can be edited in the future when the device state is updated. Editing the information of a proposed device forces the user to put it into one of the other 4 device states.
Inactive: Device is no longer in use. Size, make, model, and device type are required for an Inactive device and cannot be edited. An inactive device cannot have test work in the Control Panel.
Removed: Device is no longer in use and is no longer in the facility. Size, make, model, and device type are required for a removed device and cannot be edited. Once a device is in the removed state the device state and device details cannot be updated. A removed device cannot have test work in the Control Panel.
Replaced: Device is no longer in use, is no longer in the facility, and has been replaced with a functional device Size, make, model, and device type are required for a replaced device and cannot be edited. Once a device is in the replaced state the device state cannot be updated. A Replaced device cannot have test work in the Control Panel.
Add a Device
Devices can be added from the Hazard and Hazard Listing screens. A hazard can have many hazard devices associated with it.
- Select the ‘Add’ button from the device section
- Choose OR create a hazard that the device will be assigned to All hazards will be listed in the pop-up
- To choose an existing hazard, select a hazard from the list in the pop-up window and click “Choose Existing Hazard”
- To add a new hazard, click the “Add New Hazard” button
- Input hazard information and details in the form and select “Save” from the bottom of the dialogue box.
- To choose an existing hazard, select a hazard from the list in the pop-up window and click “Choose Existing Hazard”
Input device details and select save from the bottom of the dialogue box.
Due Date: The date that the device should be tested by. Automatically advances when a test is completed.
Device State: can be Active, Proposed, or Inactive.
BC: device barcode (customer device identifier field)
CCID: device cross connection identifier (customer device identifier field)
Serial No: device serial number (customer device identifier field)
Device Type: Can be RPZ, DCVA, PVB, or SRVB; Can only be edited on devices in a proposed device state.
Valve Type: Can be Ball valve, Butterfly valve, Gate valve, or No valve type.
Device Group: Can be Contractor, Hydrant, W&S, or None; Default is None.
Device Number: Device number (customer device identifier field)
Device Manufacturer: can be Ames, Apollo/Conbraco, ARI, Caleffi, Cash Acme, Cla-Val, Conbraco, Deringer, Febco, Flomatic, Hersey, Neptune, Watts, Wilkins

Device Model: device model for the device being tested.

Device Size: can be (1/4), (3/8), (1/2), (3/4), (5/8), (7/8), (1 ¼), (1 ½), (2), (2 ½), (#), (4), (6), (8), (12)

Confined Space: can be yes or no; defaults to no; denotes whether or not the device is in a confined space.- Once the device is added it will be visible in the Device list on the Hazard Listing screen. If the device is in an active device state, t est work will also be generated and displayed in the Control Panel.
Edit a Device
- In the device section, select the device you would like to edit from the list and click the ‘Edit’ button.
Make any edits and select the “Save” button from the bottom of the dialogue box to apply your changes.
Note: Device type, manufacturer, model, and size cannot be edited on a device in the “Active” state. Once a device is in a Removed or Replaced state its device state cannot be updated further.
Delete a Device
Deleting a hazard’s device removes the device itself as well as any existing test work. A role with Delete permission is required. Deleting a device is not recommended, changing the state to Removed, Replaced, or Inactive is recommended.
- In the device section, select the device you would like to delete from the list and click the ‘Delete’ button.
- A dialogue box will appear to confirm you wish to delete the selected device. Type ‘DELETE’ and click the ‘Delete’ button.
A pop-up will appear to confirm that the device was deleted
- Once the conformation pop-up is closed, the Hazard screen will remain open and the device will no longer be visible in the list. The device will also be removed from the Hazard Listing screen
Test
The Test section displays all tests performed for the selected hazard. The columns displayed will vary depending on the device type because the fields on the test form can vary based on device type. The tests listed in the Test grid are automatically sorted from newest to oldest and the sort order can be modified by clicking on the Test Date column.
View a Test Report
- Select a test report to view from the list and click on “View Test Report”
- A pop-up window will open and display the filled-out test form for that device
Print Test Report
- Select a test report from the list and click on the “Print Test Report” button.
- The software will open a new tab, where you can view and print the test report.
Delete Test Report
Delete permission is required for this action. Deleting a test is not recommended and cannot be undone—once removed from the software, it is irreversible.
- Select a test report to delete and click on the “Delete Test” button.
- Type ‘DELETE’ in the pop-up window and click the “Delete” button.
- A pop-up window will appear to confirm the test has been deleted.
- Once the conformation pop-up is closed, the Hazard screen will remain open, and the test will no longer be visible in the list.
Email Test Report
The test report will be emailed to any facility contacts that have the “Test Report” email notification enabled and an email address listed in their contact information. If no user has the notification enabled, the software will display a warning message and prevent the action.
- Select a test report to email and click on “Email Test Report”
- If a facility contact has Test Report notification enabled, a pop-up will appear at the top of the screen.
- To review emails that are queued and emails that have been sent, select “Email History“ from the left navigation menu
